For returning clients with new concerns who are scheduling a follow-up appointment. Clients returning after two years or more must schedule a new Complete Initial Health Consultation and complete the Complete Health Assessment Form instead.
For new or existing clients scheduling a Nutritional Deficiency Assessment & Supplement Recommendations appointment.
Recommended for new clients who are not scheduling a Complete Initial Health Consultation, including those booking a 30- or 60-minute appointment for Ayurvedic Diet Guidelines, Product Recommendations & Guidance, or another focused appointment.
